Dry Needling Therapy: A Modern Approach to Releasing Deep Muscle Pain
Dr. Pawan Yogi
MPT (Neurology) | Certified Dry Needling Practitioner
If you suffer from chronic neck stiffness, persistent back aches, or stubborn muscle knots that massage therapy cannot release, you might benefit from Dry Needling. It is an advanced, evidence-based physical therapy technique designed to target deep muscular pain directly at its source.
At Shri Shanti Physiotherapy Center (known locally as shri shanti physiothay clinic in Chirawa), we offer dry needling under the certified supervision of Dr. Pawan Yogi. In this article, we explain how this modern therapy works, its primary benefits, and what you can expect during a treatment session.
What is Dry Needling?
Dry needling is a technique where thin, sterile solid-filament needles (similar to acupuncture needles) are inserted into the skin and muscle. Unlike medical injections, these needles contain no medication or fluid—hence the term "dry."
The primary target of dry needling is a myofascial trigger point, commonly referred to as a muscle knot. Trigger points are taut, hyperirritable bands within a muscle fiber that cause localized tenderness, pain, and can even refer pain to nearby areas.
Is Dry Needling the Same as Acupuncture?
While both therapies use similar needles, their philosophies are entirely different. Acupuncture is a traditional Chinese medicine practice focused on balancing the flow of energy (Qi) along spiritual meridians. Dry Needling is a modern, Western medical treatment based on anatomy, neurophysiology, and mechanical trigger points.
How Does Dry Needling Work to Relieve Pain?
When a muscle is overused, injured, or subjected to poor posture, it develops trigger points that restrict blood flow and oxygen. This leads to an accumulation of acidic waste products, making the muscle painful and tight.
When Dr. Pawan Yogi inserts a needle directly into a trigger point, it triggers three main physiological reactions:
- Local Twitch Response (LTR): The muscle undergoes a quick involuntary contraction or "twitch." This twitch resets the muscle fibers, instantly releasing tension and lengthening the tight band.
- Restored Blood Circulation: The micro-trauma from the needle signals the body to increase local blood flow, flushing out lactic acid and bringing fresh oxygen/nutrients to accelerate healing.
- Neurological Block: The insertion stimulates large nerve fibers, blocking slow pain signals from reaching the brain (based on the Gate Control Theory of Pain), providing rapid pain relief.
What Conditions Can Dry Needling Treat?
Dry needling is highly effective for a wide range of neuromuscular conditions, including:
- Chronic neck and lower back pain
- Tension headaches and migraines
- Shoulder impingements and rotator cuff pain
- Sciatica and hip joint pain
- Tennis elbow and golfer's elbow
- Plantar fasciitis and heel pain
- Fibromyalgia and localized myofascial pain syndrome
Is the Treatment Painful?
Most patients feel little to no discomfort when the needle is inserted because it is extremely thin. However, when the needle hits the active trigger point, you will feel a deep ache, cramping sensation, or a brief muscle twitch. This "local twitch response" might feel intense for a second, but it is a positive sign that the muscle is relaxing.
After the session, you might experience mild muscle soreness, similar to the feeling after a strenuous workout. This is normal and typically disappears within 24 to 48 hours.
